Comprehensive Home Care is proudly certified as a 245D Provider by the Minnesota Department of Human Services (DHS), granting us the opportunity to collaborate with select Minnesota Medicaid Waiver Programs. Our goals align with ensuring individuals receive the necessary supportive services within environments that promote community integration and minimal restrictions. As part of our dedication to this principle, Comprehensive Home Care offers 245D HCBS services. These initiatives are designed to empower individuals and their families, fostering a sense of security, well-being, and tranquility as they continue to thrive in their own homes and communities.
